Each request is manually compared to a master Gainfield family list so please be sure to fill the in the information below completely and accurately.
*If you are a future kindergarten parent, please indicate either of the current Gainfield kindergarten teachers in the "oldest child's teacher" field. You will obviously not know who your child's teacher will be yet, so either name is fine. We require this field to be entered with a valid Gainfield teacher so as to validate those who are truly part of the Gainfield Community.
